7 FISHHAWK CDD FIELD INSPECTION REPORT June 21 & 28, 2017 Rizzetta & Company John R Toborg Sr. Field Services Manager

8 Osprey Club & Osprey Ridge, Eagle Ridge General Updates, Recent & Upcoming Maintenance Events During the month of July, all St. Augustine & Bermuda turf shall receive an application of Fe, Mn & Mg & Dominon fertilizer. Dominion is an insecticide. Throughout the community, trees need lifting. The Spec calls for 10 over sidewalks & 14.5 over roads. CLM to provide a schedule when this will be completed throughout the community. Also throughout the community, make sure all street traffic signs are kept clear of all landscape material, specifically tree branches. CLM to provide a schedule regarding the pruning of all Palm trees as well as the lifting of all street trees throughout the community. Also include a schedule for the removal of all Spanish Moss. The following are action items for Capital Land Management (CLM) to complete. Please refer to the item # in your response listing action already taken or anticipated time of completion. Red text indicates deficient from previous report. Bold Red text indicates deficient for more than a month. Green text indicates a proposal has been requested. Blue indicates irrigation. 1. CLM to perform a major leaf blowout of many landscape beds, particularly those Magnolia trees here at the Osprey Club leading into the pool area. 2. Remove any suckers on the Crape Myrtles from the pool area or elsewhere. 3. Remove the weeds from the small perimeter bed between the pool deck and fence line where only Liriope exists. 4. Also make sure weeds are eradicated out of the pool paver joints. 5. Make sure to keep the Confederate Jasmine climbing on the fence off the chaise lounges. 6. The totlot and swingset areas needs to be eradicated of all weeds. It is preferred these are hand removed as opposed to spray since they are active with children playing. 7. The Osprey Club berm bed needs to be soft edged. And weeded There is an error on the maintenance exhibit which needs to be corrected. The exhibit shows CDD to maintain to the top of the berm only, however, the CDD owns clear to the rear lot lines of the residents on the back side. This has been communicated to the vendor. 8. Within this berm, in the past a Pine tree has fallen into the Osprey Club lawn. Previous contractor had mowed around this. I ve asked CLM to remove this fallen tree and restore the area to its original condition and mow the turf as it was intended. 9. CLM to continue to remove any dead plant material. 10.CLM should try to get a head start on the stink vine which is gaining a foothold on many hedge rows. 11.At the FH fountain at the tip of the Osprey Ridge median & FH Blvd., keep the beds delineated and remove minor weeds encroaching into the Dwarf Asian Jasmine. 12.Make sure all landscape lights are kept clear of all landscaping at all times. 13.At the other end of this median at Eagle Ridge, remove taller weeds from this bed of Dwarf Asian Jasmine. 2

9 Osprey Club To Heron Park 14.All along the berm between the Osprey Club and Heron Park Villas, remove the vines from the Viburnum hedge running the length of the berm. Also work toward establishing a soft edge along this bed line. 15.Approximately 100 before the Heron Glen entrance, remove a dead branch overhanging the hedge. 16.CLM to provide a proposal to incorporate Black Kow into the beds containing Knockout or Drift Roses. 17.I will do some research regarding the remaining warranty of the (3) standard Tibouchina at the entrance to the Heron Park Villas. Two of the three are dead. If there is no warranty remaining or BV will not honor the warranty, I will ask CLM to provide a proposal to replace them, however, Tibouchina is not always fond of the full sun all day, therefore, I may make a substitution. (Pic 17) 19.At the southern end of the interior park on Heron Glen between Heronview & Heronrise, inspect the Fakahatchee Grasses for spider mite. If present, perform a thorough drenching and cut to a low mound (app ). Also focus on the removal of Spanish Moss in the trees. Also in many of the Saw Palmetto beds, periodically remove all dead fronds. 20.Heading south on Heronglen where the trail crosses, CLM needs to cut back a rather thick limb protruding into the trail posing a danger. This needs to be cut farther back from the trail. (Pic 20) And as a reminder, in the same trail as mentioned above (and all others), crews are to maintain a mow width on either side of the trail mowing and removing limbs, fronds, etc One of the Tree Ligustrum in the roundabout outside Heron Park Villas has always exhibited chronic spotting and a thinning habit. I would like to ask CLM to prescribe an on-going treatment program (i.e., fungicide and/or fertilizer mixture) to be applied. (Pic 18 >) 3

10 Heron Glen, Tealside, Kestrel Ridge, Vireo Ridge, Paloma Glade 21.We need to rejuvenate the Crape Myrtles on the median at the southern entrance into Heron Glen. These are rather anemic looking and we need to make sure they receive adequate fertilization during the March, June, October Ornamental fertilization program. 22.Remove large weeds from the Agapanthus beds on the inbound side of Teal Rise. 23.The Tibouchina on the Tealside Ct. cul-de-sac may be losing out in the water war with the Oak they are planted under. Irrigation crews should look into the water distribution here and make any necessary adjustments. (Pic 23) 27.Remove Muscadine Vines from an Oak branch overhanging the outbound side of Vireo Ridge. 28.Who should be responsible to remove the terrestrial weeds covering the aquatic plants on the southern bank of Pond 27 past Paloma Glade along the Osprey Ridge Dr.? In my opinion, this is more of an Aquatics Maintenance issue. (Pic 28) Also along this same stretch of Osprey Ridge, inspect some of the Fakahatchee Grasses for spider mite and treat accordingly as mentioned earlier in the report. 24.On the trail along Osprey Ridge between the southern entrance of Heron Glen and Kestrel Ridge, make sure limbs and vines are being cut back from the sides of the trail as well as from overhead. Many areas are being encroached upon. 25. CLM to always be cognizant of street/traffic signs being blocked by tree limbs and other landscaping, be it natural or planted, and remove that which is blocking the view. 26.Remove minor weeds at the Kestrel Ridge median. Also on the Osprey Ridge medians, make sure Saw Palmettos and other plant material is not allowed to grow into the traffic lanes. 30.Make sure all dead material is removed from the Bulbine and spent flower stalks are pulled from the Flax Lilly at the Martin Meadow entrance. 31.Check the turf on the back side of Paloma Glade Dr. cul-de-sac. It appears there is old chinch bug damage, If chinch bug are present, treat accordingly. 32.On the back side of the Loropetalum at the Paloma Glade entrance, there were plants installed that have now died. I am trying to research the install time, but assuming the warranty is expired, I will work with CLM for replacements. (Pic 32 >) 4

11 Paloma Glade, Shearwater, Merlin Glen To Sandhill Place 33.Crews need to re-visit the trail system between Paloma Glade and Shearwater and make sure the sides are being maintained and also to remove overhead material. 34.The main roundabout inside Shearwater needs to have the Dwarf Asian Jasmine bed weeded. It is infested with a mock vine. 35.I would like a CLM arborist to take a look at the Oak tree on the cul-de-sac at Shearcrest Dr. The crown is not well formed and there is also a lot of Ball Moss, which often indicates an underlying stress. Remove the dead Shell Ginger underneath this same Oak and replace (Install & Invoice). This cul-de-sac planting is not as successful as others, so let s try to tick off possible causes like irrigation issues. 39.Remove any spend flower/seed stalks from Washington Palms and remove boots from planting beds and curbs, property-wide. 40. The beds at Jaeger Glen need to be weeded. 41.Past the roundabout at Tanager Ridge, and going onto Dorman Road toward Sandhill Place, beds on both sides of Dorman need to be weeded. Most are covered with various vines Leaving Shearwater and heading north on Osprey Ridge, there are dead limbs as well as a dead tree back by the trail that need to be removed. (Pic 36 >) 37.On the inbound side of Merlin Glen, crews need to remove stink vines off Saw Palmettos as well as keep them cut back behind the curb and gutter. (Pic 37 >) Inspect the turf on the cul-de-sacs of Merlinpark Place. There is minor yellowing which might be the early stages of fungus The Sandhill Place entrance features are also loaded with weeds. Also try to maintain a consistent height in the Arboricola. Also make sure there is a clear delineation between the Blue Daze and the Dwarf Asian Jasmine beds at the front of the entrance. 5

12 Falcon Ridge, Kitecrest, Hawk Park 43.Between Ponds 75 & 76 in Falcon Ridge, crews need to make sure to maintain the hedge line that separates the first resident form Pond 75. Currently, the turf is not mowed and the hedge is covered with vines. Additionally, crews need to make sure they are mowing the entire south bank of Pond 75 including the area surrounding the dog station. The mowing also appeared to be inconsistent here with intermittent high spots. Additionally, line trimming could improve in this area. 44.Both sides of Kitecrest are loaded with weeds and unkept planting beds. The Dwarf Firebush here needs to be trimmed to a consistent height as well as periodically tipped to maintain a compact form. I would start by cutting them back by at least a third. 45.At the end of Falconcreek Place cul-de-sac, make sure all overhead and side material is being kept off the sidewalk. 46.The beds leading into Harrier Ridge at the column features are overrun with weeds. These beds also need to be delineated and pruned.(pic 46) Both roadway plantings under the Power Lines on Hawkpark Blvd. need to be detailed. Remove vines covering the Wax Myrtles, define the bed edge, tip back the Plumbago and remove Potato Vines and tall weeds. Include the bed just past the last house on the right heading toward the Hawk Park entrance. 50.Maintain a consistent height in the hedge surrounding the tennis court in Hawk Park. 51.The area surrounding the totlot at Hawk Park appears to have been missed at mowing. 52.I will ask Finn Outdoor to address the Cogongrass at the front left of the Hawk Park clubhouse. 53.The beds along the sidewalk in front of Hawk Park clubhouse ALL need attention, including the annual plantings. The Liriope bed is loaded with tall grassy weeds much taller than the Liriope itself. 54.Let s see if we can t bring the two Jatropha on either side of the clubhouse door back to life by regularly pinching it back to promote a fuller, more compact, growth habit. Also check the irrigation coverage to make sure there is adequate water. 55.CLM needs to inspect the turf to the left side of the clubhouse. There are almost perfect circular spots indicating fungus. Spot treat weeds. 56.CLM to provide a proposal to remove (flush cut) the last remaining Washington Palm at the Hawk Park Clubhouse. (Pic 56 >) 47.Leading into Harrier Ridge, tip the four beds of Dwarf Asian Jasmine under the Oaks. 48.Remove an overhanging Oak branch into the Harrier Ridge Lift Station. 57.Pool perimeter fence line plantings need to be weeded. 58.Also inside the pool deck the beds are very weedy. 6

13 Hawkwood & Hawkwood Park 59.The Tree Ligustrum at the entrance to Hawkwood needs to be trimmed at the top. 60.CLM to inspect the Viburnum hedge at the northern end of Hawkgrove Place. Several spots are turning brown. Diagnose, treat accordingly and report back findings. 61.As a reminder, make sure the Viburnum hedge south of the cul-de-sac on the south end of Hawkgrove Place is always maintained. It s a bit outta sight outta mind. 62.The same thing goes for the roadway plantings on Hawkwood Court as applied to the roadway plantings under the power lines on Hawkpark Blvd. mentioned in Item No Exiting Hawkwood at Hawkpark Blvd. on the north side of Hawkcrest Dr., make sure crews are aware of a large orange on the map tract next to the last house that is under CDD maintenance. It is currently pretty rough. There has also been a dead ornamental palm for a very long time further to the north end of this tract. CLM to provide a proposal to flush cut this palm and have it removed At the southern tip of the median leading to Lithia Pinecrest Road, fill in the bare spots of Blue My Mind Blue Daze. Make SURE to install Blue My Mind variety. Install and Invoice. 65.Remove Potato Vine from the Oleander on the outbound lanes of Hawkpark Blvd. leading to Lithia Pinecrest. Also cut back the Oleander by app. 1/3. 66.Make sure the beds of Dwarf Asian Jasmine are kept cut behind the curb and gutter on the front of the median, but also kept cut in clean lines from other plants in the same bed. 67.Remove climbing vines coming over the outbound side monument from the back. 68.Remove weeds from the beds on the inbound side of Hawkpark Blvd. 69.On the median in front of the guardhouse, there is a large open spot. CLM to provide a proposal to install seven (7) 3 Gal. Dwarf Firebush spaced app. 3-4 apart. 70.Try to keep the Cardboard Palms cut behind the curb on the outbound lanes of Hawkpark Blvd. 71.The 3-rail fence along Lithia Pinecrest west of the Hawkpark entrance needs to be detailed. Weed the beds, define the edge and remove all dead plant material. 7
