1 LANDSCAPE INSPECTION REPORT 9/4/13 CDD1 August 2013 Attending: Ronnie Hughes - Down to Earth, Eddie Tanguay IDA Operations Summerfield Park 1. Weeds in clay of baseball diamond need sprayed and edged. 2. Spray magnolia weeds leading to the playground equipment, both of them, on the left. 3. Remove the dead limb in the wax myrtle near the tennis court on the northeast corner. 4. Trim crepe myrtles and wax myrtles off the fence at the tennis court and remove the dead limbs in the juniper. 5. Along sidewalk at the soccer field, treat for bed weeds underneath oaks. 6. Remove dead pine tree limbs from the bed south of the tennis court along the sidewalk. 7. Remove dead Indian hawthornes from Parkside Place. Approximately three or four. 8. Sidewalk going west, remove the dead wax myrtle along the walkway. 9. Trim the grasses along the walkway from overhanging the walk. 10. Last oak along the walkway leading into Parkside Place, trim up the limbs to contract height. It is right over the sidewalk. 11. Crosswalk going into eastside of Park Place, remove the large oak sucker inside the wax myrtle. A little further from there is a dead wax myrtle limb that needs to be removed. 12. Night jasmine along walkway headed to Park Place, trim to give it some better shape. And trim back from the walkway if possible. And lift the last magnolia (a couple of limbs) overhanging the night jasmine. 13. Last viburnum on that walkway, trim it to get it off the walkway. 14. Remove dead palmettos in the palmetto bushes that are accessible from view. 15. Next to the play equipment, spray the weeds in the volleyball court. 16. Continue vine removal in the palmettos on the west side of the play equipment in the palmetto bushes what you can reach from the front. 17. Bed weeds around the oak trees throughout the play equipment. 18. Remove the palmetto in the butterfly garden. 19. Continue around kiddie swing spray the weeds encroaching into the playground. 20. Remove the grass and tie the bed in together going to the kiddie playground swing towards the parking lot between the two oak trees. 21. Add some pine straw between the viburnum hedge leading into the parking lot two spots. 22. Remove the two suckers under the oak tree in the middle of the parking lot. 23. The front road along Lakewood Ranch Blvd., trim the night jasmine to the wax myrtle. 24. Near the dumpster, bed out by Lakewood Ranch, spray and remove the suckers in the bed. Spray for weeds and shape the arbicola. 25. In the juniper leading to the main walkway going to the bathrooms out by the parking lot, remove the weeds under the junipers.

2 Park 26. Entering park, near light pole #1, spray for bed weeds. Also spray the weeds in the jasmine beds, same areas, along the road. 27. Entering Fetterbush, spray the weeds under the oak trees. Also edge the beds and spray the weeds under the jasmine. Edge the same bed. 28. Corner of Goldenleaf and Waterwillow, the large island, trim the night jasmine. 29. Sidewalk from Waterwillow to Clubhouse, treat for crack weeds. Edge the variegated vine growing across the walkway. Walk through area and blow off walkways. Edge plant material growing on sidewalk on the south side. 30. Cul-de-sac at Parkside Place, tighten up the jasmine in the center cul-de-sac. 31. Blue Sage, tighten the one single jasmine by the mailbox. 32. At the intersection of Buttonbush Ct and Meandering Way, trim up the crepe myrtles hanging over the sidewalk to get them off your head. That same bed, spray the weeds underneath the juniper. 33. Primrose Park treat the weeds in the beds that have been edged. Pretty much the whole park. Spray the shell path. Edge the mimosa back and trim down the plumbago and treat for chili thrips. 34. Treat all plant material in the park with insect and micronutrients. 35. Trim the gold mounds that are six feet tall down to four feet and shape. 36. Overall, the park needs some weeding, edging and clean up. Concentrate on that and palmettos will be addressed next month. Lakes 37. Starting at Deerberry, corner of Tumbleweed Trail and Meandering Way, trim the firebush to be the same throughout the bed, edge and spray weeds. 38. Summerfield Lakes entrance, remove the oak suckers inside the Indian hawthorne. 39. Something growing in center island in the holly. 40. Treat for weeds under junipers and in the junipers. Woods/Bluffs 41. Apply amodium sulfate to the duranta at the entranceway to help fill in. 42. Trim the night jasmine at the end of Pine Lilly Place. 43. East corner of Wild Flower & Clubhouse Dr, trim the oak up above the stop sign. Starting to encroach. 44. Along wildflower beds under the oak trees, pickup sticks and spray for weeds on the east side of the road. 45. West cul-de-sac on the end of Pine Lilly Place, remove the pepper tree growing out of the fern. 46. Remove the dead in the palmettos near the Summerfield Bluffs sign entrance between River s Bluff Circle and Pine Lilly Place in little park area. Treat the beds for weeds and edge. 47. West of the Lakes entrance, lift the crepes and oaks to contract height above the sidewalk on north side of road, just east of lightpole # Across from lightpole #8 on the north side of the sidewalk, trim the firebush back off walkway. 49. Also the philodendrons headed towards lightpole #5, trim back off the walkway.

3 50. The River Club entranceway, the Lakewood Ranch sign on both sides (newly installed) need weed control. 51. Headed east on Clubhouse on the south side of the road, put an edge and remove weeds and vines in the palmettos and other. 52. Headed east before the pedestrian sign, good sized limbs, pick them up you can see from the road. Large limbs (not sticks) looking out at the preserve. 53. Vines in the junipers headed into corner of Wildflower Lane and Clubhouse Dr. Opposite end, east side of same entranceway, treat for weeds in juniper and whack down the suckers underneath the oak trees. 54. Trim back some of the firebush headed east towards lightpole # Trim back the fern near lightpole #10 and philodendrons overhanging the grass edge. Hard edge as needed. 56. Across from lightpole #9 (or 5) trim up the firebush over the grass edge. 57. East of lightpole #8, trim back the firebush overhanging to the road. 58. Bed weeds in the juniper west of Woods and Bluffs sign. 59. Light pole #4, remove the rogue vine running rampant through the berm. 60. Remove the vine growing up inside the Viburnum west of the lift station on white fence. 61. Vines in the ligustrum trees at the four way intersection of Clubhouse and Lakewood Ranch on the south side. 62. North side of Summerfield Glades treat for bed weeds. 63. Touch up mulch headed north near the light pole same intersection. 64. Spray the weeds headed north on the east side of the intersection for broadleaf weeds and add potassium to strengthen roots. 65. Remove the large limb same berm. 66. Trim back the bougainvillea over the sidewalk same area. 67. South side treat grassy weeds. Remove Virginia creeper under the juniper and in the flowers Remove sticks behind monument sign on south side. 69. Check on the pump station for the vine that is on the fence who s responsible? 70. Misty Flower Glen cul-de-sac trim the carissa off the crepe myrtles and do a light pruning to give them shape. 71. At the end of Summer Meadow the cul-de-sac vines in the cypress and trim back off grass area. Give Ronnie a little extra time to accomplish the satisfied results we want. Add micros to that cul-de-sac. 72. Softrush Terrace cul-de-sac, cypress need to be trimmed up. Re-define the bed line in the same area cypress trees along the berm. Same thing: we ll be considerate and let Ronnie have a couple of months to get it completed. 73. Summer Blossom Lane near the pond, the jasmine bed needs weed treatment. 74. Intersection of Summer Blossom Lane and Clubhouse, trim the jasmine, edge the bed and spray for weeds. 75. Blackberry Lane near the pump lift station, trim the night jasmine and the vine growing up the oak tree. 76. Treat the shell at the pump station for weeds.

4 77. Treat the nut sedge weeds in the turf area headed south of Wood Sage Terrace and Blackberry on Blackberry. 78. Cul-de-sac at Barberry Court treat crack weeds, trim the jasmine, spray the roses with micronutrients and pest and disease control. General Note: Trim back both sides of the road to at least the grass edge on the east side. Forest Place 79. The entranceway to Place, lift the oak on the east side at the corner of Whistling Way. 80. On the east side, trim back the firebush at the corner on the exit side to the edge of the grass. Green 81. Knock down the weeds on the west side and the crepe suckers on the berm, pickup the sticks. 82. Remove the vine in the juniper in center island at the monument. 83. Cul-de-sac at Hollyhock Dr, treat the St Augustine with micronutrients and potassium to help recover from the chinch bug damage. 84. Exit side of Green, treat the bed weeds and shape the jasmine. Glen and Pines 85. Longleaf Pine Court raise the crepe myrtles and remove the low hanging branches on the crepes. 86. Tighten up the Buford holly around the oak tree at the end of Winding Woods Way at the west culde-sac. Roserush Ct Linear Park 87. Roserush Ct vines in the juniper and grassy weeds. Also some crack weeds. 88. Popash Glen treat the turf in cul-de-sac for weeds. Juniper has grassy weeds also. 89. East side of Linear Park, trim the jasmine around the crepe myrtles at the sidewalk. Spray turf weeds. Mulch beds and add some additional pine straw. 90. Line trim the oak tree suckers at the pond, east side and remove dead branches. 91. Remove additional oak suckers in the first oak tree headed west on the bark. 92. Trim night jasmine around the first bench along walkway. 93. Treat the dollar weed in the turf areas west in the Bermuda. 94. Treat the caterpillars on the oleander on the west side of Winding Woods. 95. West side of linear park trim the firebush to consistent height at the far west end of the sidewalk. 96. Headed back east, remove the Virginia creeper off the pine tree north side of the sidewalk. 97. Treat the grasses from blight and pick up additional sticks that have dropped. 98. Trim the night jasmine throughout the park. 99. Trim the oak tree suckers in the beds coming off Linear Park at Roserush Ct entrance.

5 100. Cul-de-sc at Blue Flower Ct spray weeds under juniper and remove sucker under crepe myrtle. Crack weeds need to be treated Acorn Woods cul-de-sac, treat the grassy weeds in the jasmine. Also sedge weed and spurge. Crest 102. Treat the crack weeds on Mosswood Place Ct cul-de-sac. Treat the sedge grasses in the jasmine. Strand 103. East of the Strand entrance at Heron s Crest, treat crack weeds. Treat the weeds in the shell path shell parking lot and under the oaks Weed control underneath the philodendrons headed north from the Strand entrance Treat the turf weeds in the east side of the roadway Nightshade Place cul-de-sac crack weeds, treat the sedge grasses in jasmine. Trim the jasmine off the curb. Remove the vine in the holly and Virginia creeper in the holly also Rockrose Glen cul-de-sac treat sedge grasses in the jasmine, oak suckers in the juniper, weed the jasmine down Whiteclover Park treat jasmine for grassy weeds. Holly trees getting into the park area west end of the park lift them so they stop encroaching on the walkway Tighten the holly in the south side of the park Treat the turf weeds in the mailbox area most south island. Also trim those holly s and crack weeds Treat the Bermuda grass with micronutrients to make it greener. Carry Over Item: End of Tall Pines Way/Foxglove Ln, weeds in juniper bed. Summerfield Parkway 112. Headed east from Lakewood Ranch Blvd, trim back the morning jasmine and remove vines Virginia creeper underneath the magnolia on the south side - bed weeds Light pole #26 near center island remove weeds in the jasmine and vine growth in the palmettos between 24 and 26 Virginia creeper West of light pole #24 treat jasmine for weeds Remove large, dead limb east of light pole # 20 on the berm Trim ferns back after the Crest entrance over the jasmine on the berm Spray the weeds in the Indian hawthorne. Treat the Indian Hawthorn with micronutrients and spray for chili thrips Remove large limb under light pole #16 in the turf area Weeds near the curbing around the jasmine in the center island after Summerfield Place headed east and west East of the light pole #10 in the palmetto bed remove the weeds and few dead limbs that you can see from the street East sidewalk headed from Glen Pines trim the firebush back a little bit off the walkway Additional vine removal behind light pole #6. Pull the vines out of the crepe myrtles.

Spray the weeds under the philodendron just east of light pole # West of light pole #4, vine growth behind the philodendrons On the intersection of Lakewood Ranch Blvd and Summerfield Parkway on the north side remove stink weed growing up in the juniper On Palmbrush at the corner of Lakewood Ranch Blvd remove crepe vine growing on the walters viburnum Continue headed west is also Virginia creeper growing in the viburnum Trim oak away from dead end sign Spray for crack weeds Trim the viburnum hedge at the corner of Palmbrush Trail and Rose Finch Ct Trim the holly behind the sign at Summerfield Hollow Trim back the sage to give better view of the sign. Knoll 134. At the monument sign, trim down the variegated abricola a little lower so you can see the sign better. Hammock 135. Center island treat the bed weeds Put an edge on the sod Check irrigation. Water is running over the curb. Adjust irrigation to water less Trim the suckers off the ligustrums in the center island End of Hyacinth cul-de-sac lift the oak to contract height in the center island Treat turf for broadleaf weed Between River Club and Hyacinth buffer area the wax myrtles remove stink vine. Edge both sides of the bed, spray weeds and remove large weed At the bridge at the Braden River nature trail trim back further the pepper tree growing over the rails of the bridge and remove the vines. Meadow 143. Remove the ropes on the mailbox at the large island Blue Stem Circle and Arrowhead Run behind the mailbox Second Meadow entrance to the south entering the Oaks north west bed - treat for weeds in bed. Oaks 145. At the Pergola island treat for turf weeds Bed weeds throughout the Pergola island. Grove 147. Tighten up the protocarpus in the center island Remove the Virginia creeper going up the cypress tree as you enter before the first house two of them.

7 149. Spoonflower cul-de-sac treat for turf weeds. Trim down a couple of curb weeds on the north side Raise the ligustrum at the benches at Sweet Flag park Remove the pygmy date behind the bench Treat the dollar weed in the St Augustine northside of the sidewalk in Sweet Flag park Treat the weeds under the oak tree in the park throughout Bluebell Ct treat the cul-de-sac for crack weeds South end of Spikerush cul-de-sac crack weeds & spray the turf for micronutrients and weeds North end next to Lakewood Ranch Blvd edge the berm and spray for weeds along the edge/re-define the edge In the cul-de-sac treat the grasses in the juniper and the crack weeds On light poles on the corner of Moonflower Way remove the jasmine that is over the glass part of the light post. Ridge 159. Remove the bed weeds underneath the sweet gums on the south side of the road In the cypress trees at the pump station remove the Virginia creeper up in the trees. Vine growth On Goldenaster cul-de-sac replenish some of the mulch on the edge On Honeysuckle and Lobelia island tighten the gold mound and add some micronutrients to the magnolias Treat for bed weeds underneath the magnolias. Crack weeds in same area Treat crack weeds same island Lobelia Terrace remove the dead crepe myrtle at the fence Headed west along the berm treat the edge for some weeds in the center cul-de-sac Pick up dead palm boots and seed pods and lower the taller ty plants 168. Staggerbush Glen cul-de-sac remove seed pods, trim jasmine over curb, spray grassy weeds in jasmine. Edge the jasmine at the fakahatchee grass and treat for blight. And cut and trim back the fakahatchee after spraying End of Honeysuckle berm underneath the fakahatchee grass treat for weeds Remove lower suckers on crepe myrtles. Hard edge the berm/bed definition, spray the weeds Remove the limb sticking out of the cypress trees on the berm Hidden River Trail middle island dead wood, seed pods, trim the juniper off curb Treat the weeds in center island. Remove suckers north of that same light pole off the bottom of the crepes North of light pole #12 tree ring spray, edge and pull weeds North of marker 25, whack down the weeds under the pine tree bed. General Comment: Spray crack weeds in the curbing down Lakewood Ranch Blvd. Heron s Nest Park Trail System 234. Right at marker 21 trim back a little further the Brazilian pepper pushing it back off the path Spray the shell path going to the bus stop in the trail system.

10 Carry-over Items: 1 Replacement items: 0 Performance Evaluation: 9/4/13 The inspection of CDD 1 this month found the property to be in good condition. The plant material overall is healthy and growing vigorously except for a few areas of pest. The turf in most areas is performing nicely considering the fertilizer blackout period and not being able to apply nitrogen to the turf. There were some areas of turf that with the amount of rain has sprouted sedge weeds which will be corrected. The annuals in most areas are performing well. The bed weeds are still a problem with the rain coming almost daily causing any chemical treatment to get washed off before it can become effective. With that said, the weeds are still being managed at an acceptable level. The vine growth has been addressed and exotics are under control. The trail system has had a few areas that are getting overgrown and will need to be monitored closely. The new mulch was installed at Summerfield Park around the facilities and play ground equipment. These areas look very nice and manicured. The new sod in the soccer field is doing well also. The plants in many areas are growing over the walks and roads and will need to be trimmed back. With 1 carry over items from the prior inspection I find the District acceptable and provided a passing grade. 12 LAKEWOOD RANCH COOl EXECUTIVE SUMMARY SUBMIT DATE SUBMITIED BY BOARD MEETING DATE I September 18, 2013 I Steve Zielinski I September 19, 2103 I ISSUE: I Authorized Account Signatures I BACKGROUND: A re-organization of the CDDl Board is to occur at its official meeting on September 19, As a result, an update of account signatures on file with the District's banking institution, Northern Trust, would be required. The recommendation proposed by the Chief Financial Officer was to include the CFO as an authorized signature on the account. This would facilitate the check signing process without compromising any internal control issues. During the interim period of September through December 2013, the District will maintain its current policy of having two Board members countersign all checks (the District Treasurer and District Chairperson). Effective January 1, 2014, the standard countersignatures required on checks for the payment of invoices would be that of the District's Treasurer and that of the Chief Financial Officer. In the event the District's Treasurer is unable and/or unavailable for check signing at any particular time another Board Supervisor would be designated to fill in during the District Treasurer's absence. New signature cards will be available at conclusion of the Board meeting of September 19 th for each of the Board Supervisors and the CFO to execute accordingly.
