1 DFR Pre-Incident Planning Frm Instructins fr Dwntwn Dallas Inc Thank yu fr taking the time t help us prepare in advance fr emergency incidents at yur prperty. The infrmatin yu prvide will be available t first respnders when they respnd t calls at yur business. The infrmatin will nt be used fr fire cde enfrcement. The frm fields in Red are fr Fire Department persnnel t fill ut and shuld be skipped. This includes: Water Supply (page 4) and Strategies & Cnclusins (Page 9) Many f the questins are self-explanatry and thus are nt cvered in these instructins. All fields that yu d nt knw the answer t can and shuld be skipped. It is imprtant nt t guess any f the answers. Blank fields are preferable t incrrect fields. Yu shuld be able t fill ut this frm n yur cmputer and then return it via . This is greatly preferred because we can then simply review and uplad the frm t ur database. If fr sme reasn that is nt pssible, yu may print the frm and write yur answers n the hardcpy. Asset Name & Address Asset Name is typically the same as the business name. If yur campus has multiple unique buildings, yu may r may nt be asked t fill ut a frm fr each building; these situatins are handled n a case by case basis. If yu are asked t fill a frm fr separate buildings at the same campus the asset names will need t be mre specific, such as Acme Prducts Warehuse, Acme Prducts Office Twer and Acme Prducts Factry Building.
2 If yur business has multiple lcatins it will be helpful t prvide a unique business name. Instead f Krger, yu culd use Krger Mckingbird-5550 r Chase Bank Rss Cntacts This infrmatin yu list here will be used t ntify yu and/r members f yur building management team when there is an incident n yur prperty. It may als be used by firefighters t cntact yu r yur persnnel t schedule a site-visit. Building Sectin Entry Key Bx Lcatin: Sme businesses have what is called variusly a Knx Bx, Lck Bx, r Fire Department Key Bx. This is a small metal bx which may be munted either inside r utside the business which cntains keys t building drs and elevatrs. It can nly be pened with a special key carried n fire apparatus. If yur building has ne f these bxes, use this field t describe where it is lcated. i Primary & Secndary Fire Dept Entrances: Sme large lcatins may have specific lcatins that first respnders shuld reprt t when there is an emergency and that lcatin is nt always the main entrance. An example f a primary entrance fr 5215 Smewhere Street might be Main Entry at 5215 Smewhere Street r Frnt Entry but less bvius examples are Suth Entrance n Yung street, Security Office n west side f building r Guard Shack at 8218 Brexit Avenue Descriptin f Building: A brief verview in yur wn wrds. Fr instance: 4 stry building with cmmercial with 3 flrs f wd framed apartments ver 1 flr f cmmercial. 1 stry flat rfed strip shpping center with 5 units 32 stry Type I Fire Resistive (steel cnstructin with fireprfing) ffice building Access/Egress Cncerns: If there are bstacles that will make it difficult fr fire apparatus t reach the scene f the emergency, yu may list them here. Examples: The building is in the rear f the cmplex and accessible nly by a single lane rad. If ne fire apparatus enters laying hse, thers will have t drive ver it. The building is in the rear f the cmplex and there are tight turns thrugh a parking lt with cvered parking. A large fire truck may nt be able t negtiate the turns.
3 Occupancy Occupancy Type: Chse the clsest fit frm the drp-dwn bx: Assembly: An Occupancy used fr gathering f 50 r mre persns. Used as a special amusement building regardless f ccupant lad. IFSTA=Assembly NFIRS=Assembly Business: Occupancy used fr the transactin f business ther than mercantile. IFSTA=Business NFIRS=Mercantile, Business Educatin: Schls IFSTA=Educatin NIFIRS=Educatinal Factry: Occupancy in which prducts are manufactured r in which prcessing, assembling, mixing, packaging, finishing, decrating r repair peratins are cnducted. IFSTA=Factry Industrial. NFIRS=Industrial, Utility, Defense, Agricultural, Mining, Manufacturing and Prcessing Institutin: Hspital, Nursing Hmes, Assisted Living, Child and Adult Day Care and Prisns. IFSTA=Institutin. NFIRS= Health care, Detentin & Crrectin Mercantile: Used fr the display and sale f merchandise. IFSTA=Mercantile NFIRS=Mercantile, Business. Residential: Prvides sleeping accmmdatins fr purpse ther than healthcare r detentin Strage: An Occupancy used primarily fr the strage r sheltering f gds, vehicles, animals, etc. Mult-Occupancy: Nt classified in any specific ccupancy. Occupant Interir Evacuatin Area: If yu have a designated area where ccupants are directed t shelter in place in the event f an emergency, describe that lcatin here. Occupant Exterir Evacuatin Area: If yu have a designated muster pint fr building ccupants t g t after evacuating the building, describe that lcatin here. Fr instance, n the nrth side f the building, acrss Main Street in the parking lt at the crner f Walk and Dn t Walk. Salvage Pririties: If there are ne r tw things that wuld be a pririty t prtect frm fire r water damage, yu can list them here. Example: Art Cllectin n first flr, west wing, Rm 310 Data Center n 5 th flr.
4 Special Needs Fire Cntrl Center Sme large buildings have a designated Fire Cntrl Center. This is distinct frm a riser rm r fire pump rm. It is a rm that typically will have a Fire Alarm Panel at a minimum, and likely will als have ne r mre f the fllwing Landline telephne Elevatr Panel Ventilatin Cntrls Public Address System Keys Cpies f building plans and pre-incident plans Cmmunicatins Systems Des the building have an Enhanced Radi Cmmunicatins System? This is a repeater system that will allw fire department radis t transmit and receive thrughut the building. With the pssible exceptin f sme new high rise buildings, the answer t this is almst always N. Alarm Systems Lcatin f Main Fire Alarm Panel: In high rise buildings it will ften be in the FCC (Fire Cntrl Center). In ther buildings it will ften be in an ffice, a lbby r in the riser rm. Building Infrmatin Building Infrmatin Atrium Architecture: If the lbby is pen t upper flrs, as is ften seen in newer htels, the answer is yes. Otherwise, n. Flr Plans Attached: If, in additin t this frm, yu are sending flr plans yu may check this Yes. Flr plans need nt be architectural drawings; in fact simple diagrams shwing hw the building is layed ut and where key features are greatly preferred. Building Cnstructin Type Type I Fire Resistive (Prtected Steel. Cmmn in high rises) Type II Nn-Cmbustible (Unprtected Steel r Cncrete) Type III Ordinary Cnstructin (Hadite blck walls)
5 Type IV Heavy Timber (Nt cmmn in Dallas) Type V Wd Frame (Typical 2x4 cnstructin). Mst apartment cmplexes being built tday wuld be Type V, even if the first flr is Type II. Is High Rise? If the building is 4 flrs r mre, answer Yes. (Fr legal purpses and fire preventin inspectins, a high rise is a building 75 r taller, but this questin is fr fire peratins purpses) Can any windws be pened r remved? This questin nly applies t high rises. Nn-high rise buildings shuld leave blank. Lcatin f remvable windws This questin nly applies t high rises. Nn-rise buildings shuld leave blank. Structural Rfing Structural Members: Use the drp dwn t select the best chice. Rain Rf? This is rare, but describes structures riginally built with a flat rf, typically tar & gravel, which at sme later date had a pitched rf added ver the flat rf, usually due t prblems with leaks. These present special challenges fr firefighters. Flring Basement Fire Walls & Partitins A standard fire wall is a blank, parapeted, nn-lad bearing (freestanding, duble, r tied firewalls), masnry fire wall cnstructed f material with at least a 4 h fire resistance rating. A fire wall is a parapeted wall, cnstructed f material with at least a 3 h fire resistance rating. Fire barriers have frm 2 h 3 h fire resistance. The wall extends frm the flr t the bttm side f the rf r flr deck, r frm the flr t a fire resistance rated flr-ceiling assembly f equal fire rating. Fire partitins have frm 1 h 2 h fire resistance, extend frm the flr t the ceiling, but in all ther respects are similar t fire barriers.
6 Fire Prtectin Systems Sprinkler System If a single zne system, put Only fr the zne name. Type: Use the drp-dwn t select the best type. The vast majrity f systems in nnindustrial applicatins will be either Wet r Dry. System Pressure, aka Churn Pressure: If there is a fire pump present, this is the pressure it delivers t the sprinkler system. Sprinkler Fire Pump Lcatin: Nn-high rise buildings may nt have a fire pump. Leave blank if the system perates n city water pressure. Standpipe System Pressure Reducing Valve (Y/N): These are mechanical valves designed t reduce the pressure at the standpipe utlet and are cmmn in high rise buildings. Pressure Reducing Device (Y/N): Sme lder high rise buildings may nt have true Pressure Reducing Valves, but instead have a fixed rifice device in the standpipe utlet which reduces pressure n a flwing hse line. System Pressure, aka Churn Pressure: If there is a fire pump present, this is the pressure it delivers t the standpipe system. Standpipe Class: I = 2.5 hse utlet; fr firefighter use nly. II = 1.5 hse utlet with 1.5 hse and nzzle; fr citizen use nly III = Cmbinatin system with 1.5 hse fr citizen use and a 2.5 utlet fr firefighters t cnnect t. Water Supply Sectin Fr DFR use nly Stairwells Sectin D nt cmplete this sectin if yur business is an apartment cmplex f 2 t 3 stries with pen stairwells. This sectin is primarily fr buildings with enclsed stairwells. Stairwell Name: This shuld be the same name n the stairwell placard if there is ne. Lcatin: Select frm the drpdwn list the general area f the building in which the stairwell is lcated.
7 Pressurized: Many high rise buildings pressurize escape stairs when the fire alarm activates. Enter Yes/N if knwn. If yur lcatin has t many stairwells t enter, at a minimum please nte the nes with rf access. Exit t lcatin. Examples: Lbby Exterir, Rss Avenue side f building. Elevatr Sectin Elevatr r Bank Name: Fr large buildings, yu d nt need t enter inf fr each individual elevatr, but instead enter them as banks f elevatrs. Fr example: Lw Rise Bank, Mid Rise Bank, High Rise Bank Blind Shaft: If an elevatr services flr 1 and flrs in a 50 stry building, then it is a blind shaft elevatr, meaning there is a shaft with n penings t the flr at least n flrs 2-15 and maybe n flrs 25 and higher. This is relevant t the fire service when peple are stuck in the elevatr. Machine Rm Less: Als called Gen2, cmmn in newer elevatrs. Utilities / HVAC Sectin HazMat Sectin HazMat On Site: Answer Yes if yu have reprtable quantities f Hazardus Materials and are required t file Tier II reprts with the state. Als answer yes if yu have dangerus chemicals and are unsure f yur reprting requirements. D nt answer Yes if yu simply have small quantities f chlrine fr a swimming pl, r paint prperly stred in a lcker, etc. EHS Facility: Only answer yes if yu have chemicals that are n the Extra Hazardus Substances list. This is rare. Strategies / Cnclusins Fr DFR use nly.
