1 2/6/14; ; PROCEDURE Applicatin This prcedure shall apply t. 1.0 Purpse The purpse f the Safety Management Plan is t ensure a safe envirnment fr staff, persns served, and visitrs thrugh preventative and rutine maintenance f facilities and equipment, including agency vehicles; and t establish a prcess fr identifying ptential safety issues, reprting incidences f injury r prperty damage, and addressing safety related cncerns. 2.0 Plan Mnitring and Evaluatin The Chief Financial Officer (CFO) is respnsible fr mnitring the implementatin f the Safety Management Plan n a regular basis and fr annually evaluating the plan s verall effectiveness and recmmending imprvements t the Chief Executive Officer as apprpriate r needed. Individuals frm the fllwing external parties shall be cnsulted as needed t ensure safety: Fire Department Health Department Insurance Safety Engineer 3.0 Facility and Equipment The CFO r designee shall be respnsible fr crdinating maintenance and care f the facility and grunds. Parking lts, sidewalks, and steps shall be kept clear f debris, ice, and snw. Cracks and crevices shall be repaired as needed t ensure safe egress. Inside hallways and walkways shall be kept neat and clear f debris; lse r frayed carpeting shall be repaired r replaced as necessary. Rutine maintenance prblems shall be reprted t the CFO. Outside technicians r experts shall be cntacted thrugh the landlrd.

2 2/6/14; ; Agency wned equipment, including chairs, desks, cabinets, etc., shall be maintained in gd cnditin. Any damaged, frayed, r brken cmpnents shall be reprted t the CFO. Unsafe equipment shall be taken ut f service until it can be repaired r dispsed f prperly. 4.0 Educatin and Training Staff shall receive training n safety at rientatin and annually thereafter. Trainings shall minimally fcus n the safe use f fire extinguishers, reprting f a fire, and fire evaluatin. The Department shall maintain recrds n fire safety related trainings. 4.1 Staff are nt required t use fire extinguishers in the event f a fire as persnal safety is the agency s first pririty. Staff shall assess the situatin and make a determinatin as t whether they can safely attempt t put ut the fire thrugh the use f a fire extinguisher. If there is enugh smke t hamper visin, staff shall nt attempt use f a fire extinguisher but shall immediately reprt the fire and evacuate the building. Training fr use f fire extinguishers is based n the PASS methd: Pull: Hld the extinguisher firmly upright. Pull the pin n the handle. Aim: Aim extinguisher nzzle at the base f the fire. Stand six t eight feet frm the fire n clser. Squeeze: Squeeze the handle t release the extinguishing agent. Sweep: Sweep slwly in a side-t-side mtin at the base f the fire and mve frward as the flame subsides. 4.2 Upn hearing the fire alarm, staff shall immediately check their area fr any persns served r visitrs, clse any windws and drs, and prceed t the nearest exit. Staff, persns served, and visitrs shall prceed t the designated gathering area lcated at the west crner (the crner ppsite f the garage) f the Inia parking lt, the nrthwest crner f the Belding parking lt, and the east parking lt acrss the street in Prtland. When exiting the building the receptinist shall bring sign-in sheets and ther items needed t identify staff, persns served, and visitrs in the building. Rll

N ne shall be allwed t re-enter the building until the all-clear signal has been given by apprpriate persnnel r the CFO. If evacuatin frm the building is nt pssible because f smke r fire, staff shall remain calm and cnsider the fllwing: Clse the rm dr. Stuff clthes r ther material under the dr t stp smke. Open a windw fr air (it may have t be brken). Exit thrugh the windw if pssible. If nt, mark the windw t shw lcatin fr fire fighters. Stay clse t the flr t avid smke. Make nise s peple utside knw that there is smene in the rm. 5.0 Fire Drills Fire drills shall be cnducted at least annually t ensure the safe evacuatin f ccupants frm the building. Drills shall be cnducted at different times during the wrk day and withut advance warning. Lgs and evaluatins f drills shall be kept by the CFO. 6.0 Fire Safety Equipment The alarm system shall be tested annually at the time f each drill. Emergency lighting and illuminated exit signs shall be checked mnthly by the maintenance staff. Internal inspectins shall be cmpleted mnthly by maintenance staff. Fire extinguishers shall be inspected mnthly by the chairpersn f the Health & Safety Cmmittee r designated persn. Fire extinguishers shall als be inspected and tested annually by a fire extinguisher service and recrded n the fire extinguisher tag. A sufficient number f fire extinguishers shall be maintained in wrking rder in every lcatin designated, accrding t Fire Safety Cde.

4 2/6/14; ; Recrds f these inspectins and tests shall be maintained by the CFO r the chairpersn f the Health & Safety Cmmittee. 7.0 Incident Reprting A critical incident is defined as any actin that results in the injury f a staff member, cnsumer, r any ther persn. All staff accidents, resulting in injury r having the ptential fr future injury t thers, ccurring within the building r while a staff persn is n duty shall be immediately reprted t their supervisr and t the Department fr dcumentatin. If injuries are apparent and f a serius nature, immediate medical attentin shall be sught. If injuries are nt apparent, staff, in cnsultatin with Medical Services staff, shall determine the need fr a medical examinatin. Human Resurces staff shall crdinate wrkers cmpensatin and shrt-term disability benefits when applicable. All cnsumer accidents shall be reprted as established in the agency s Unusual Incident Reprts prcedure. All Incident Reprt frms shall be reviewed by the apprpriate staff and frwarded t the Office f Recipient Rights fr lgging and safekeeping. 8.0 Vehicles The vehicle custdian shall cnduct thrugh inspectins f agency vehicles each mrning during the regular wrk week. Preventative and rutine maintenance f vehicles shall be cmpleted by vehicle custdian r external technicians as needed. Inspectins and maintenance recrds shall be maintained by the vehicle custdian fr each agency vehicle. Agency vehicles are fr business use nly. Staff and all passengers shall have their seatbelts fastened at all times. Staff shuld adjust the seat psitin and mirrrs prir t leaving the parking lt. All vehicles are "N Smking" envirnments and shall be returned clean after use. Staff shall reprt any safety cncerns with vehicles t the Chief Financial Officer r his/her designee. 9.0 Vehicle Accident Reprting

5 2/6/14; ; Vehicle accidents invlving staff r persns served and ccurring in agency vehicles r staff s persnal vehicles during wrk hurs shall be reprted immediately t the CFO r designee (with injuries t be reprted t Human Resurces Department as utlined abve). The plice shall be called and an accident reprt cmpleted. Staff r persns served injured in the accident shall g t the nearest hspital emergency rm fr bservatin. The CFO shall instruct staff making arrangements fr twing and repairs if necessary. All needed repairs shall be investigated by the CFO and apprved by the Chief Executive Officer Reprting Findings f safety inspectins, bth internal and external, shall be reprted n an annual/semi-annual basis. The reprt findings shall include the status f the safety area, including any areas that require change, and the steps t be taken t implement the changes. Additinally, all critical incidents regarding safety issues shall be included in these annual/semi-annual reprts Dcumentatin Health and Safety Cmmittee will receive events, incidents and cncerns f any issue f a safety nature. The event, incident r cncern will be dcumented in the Health and Safety minutes Remedial Actin Health and Safety Cmmittee will review events reprted as a safety cncern. Health and Safety Cmmittee will give directives r recmmendatins t apprpriate entities t decrease r reslve the safety cncern Debriefing f Critical Events When a critical event ccurs, an incident reprt will be cmpleted. The incident reprt will be frwarded t the supervisr. The supervisr will determine if debriefing is necessary and will meet with the emplyee immediately if needed. The supervisr will dcument the debriefing n the incident reprt.
