1 HOT WORK PERMIT PROGRAM SAMPLE POLICY OBJECTIVE: T establish requirements fr perfrming ht wrk during maintenance and cnstructin activities cunty facilities. POLICY: All cunty emplyees and utside cntractrs wh may be invlved in ht wrk activities shall cmply with all aspects f this prgram which is in cmpliance with OSHA 29 CFR (Welding, Cutting & Brazing) and NFPA 51B (Standard fr Fire Preventin During Welding, Cutting, and Other Ht Wrk). SCOPE: This prgram is designed t prevent injury and lss f prperty frm fire r explsin as a result f ht wrk in cunty facilities, spaces and activities. The prgram cvers welding, trch cutting, brazing, sldering, grinding, heat treating, pwer-actuated tls, thawing frzen pipes by trch, trch applied rfing, and all ther applicatins prducing sparks, flames, r heat. RESPONSIBILITIES: MANAGEMENT: - Management is respnsible fr the safe peratins f ht wrk activity: Designate a Permit Authrizing Individual (PAI) (Safety/Lss Preventin/Risk Management). Establish permissible areas fr ht wrks. Ensure nly apprved equipment is used. Ensure equipment is safe fr use and prperly repaired by a qualified persn befre use, when repairs are needed. Ensure emplyees and cntractrs are familiar with the standard.

2 Ensure emplyees are trained and aware f the risks and emergency prcedures. Advise cntractrs f any fire hazards and risks. PERMIT AUTHORIZING INDIVIDUAL (PAI) - PAI (Safety/Lss Preventin/Risk Management) is respnsible fr: The safe peratin f ht wrk activities. Determining site-specific flammable materials, hazardus prcess, r ther ptential fire hazards in the wrk area. Cnsidering alternatives with management. Nt allw ht wrk if: The wrk cannt be mved t lcatin free f cmbustibles. Cmbustibles cannt be mved t a safe distance r shielded against ignitin. Determine that fire prtectin and extinguishing equipment are prperly lcated at the wrk site. Ensure that a fire watch is at the site (if a fire watch is needed). Check ½ hur after the cmpletin f the wrk fr smldering fires (if a fire watch is nt needed). Pst the permit at the wrk site. HOT WORK OPERATOR Ht Wrk Operatr is respnsible fr: Handling ht wrk equipment safely and nt endanger lives and/r prperty. Getting PAI s apprval befre starting ht wrk activities. Examining equipment t ensure it is in safe perating cnditin. Stpping wrk and ntify PAI and management if unsafe cnditins develp. FIRE WATCH Fire watch shall have fire extinguisher equipment readily available and be trained in its prper use. Be familiar with facility and prcedures fr sunding the alarm system in the event f a fire. Watch fr fires in all expsed areas. Try t extinguish fires within the capabilities f the extinguishing equipment and nt putting themselves r thers in danger. CONTRACTORS

3 Befre starting any ht wrk, cntractrs and the cunty shall discuss the prject cmpletely including the type f ht wrk t be cnducted and the hazards in the area. Cntractrs must ensure that their emplyees are trained in the prper prcedures fr ht wrk. MUTUAL RESPONSIBILITY Safety in ht wrk activities shall be recgnized by: Management Cntractr PAI Fire Watch Ht Wrk Operatr FIRE PREVENTION PRECAUTIONS Ht Wrk may nly be perfrmed in: Permissible Areas Areas that are r have been made fire safe Designated Areas Areas apprved fr ht wrk (welding shps) Permit Required Areas Areas made fire safe by remving r prtecting cmbustibles frm ignitin Nn-Permissible Areas Areas nt authrized by management fr ht wrk activity HOT WORK PERMIT A written Ht Wrk Permit (Appendix A) shall be required befre ht wrk activity begins in a nn-designated area; the permit will be prepared and issued by the PAI. Befre the permit is authrized: The wrk area must be made fire safe. Equipment must be in safe perating cnditin. Cmbustibles must be mved r cvered with fire retardant r nncmbustible material.

4 Area must be swept clean f cmbustibles r kept wet fr a 35 ft radius (ht wrk peratrs must use prper PPE t prtect themselves frm pssible shck if flr is t be kept wet). Cracks in walls, flrs, r ducts within a 35 ft radius must be cvered with fire retardant r nn-cmbustible material. Shield r guard cmbustible walls, partitins, ceilings, r rfs. Fully charged and perable fire extinguishers, apprpriate fr the type f fire shall be readily available. If sprinkler heads are clse t the ht wrk, a wet rag shall be laid ver the head and remved when ht wrk is cmpleted. The PAI shall determine the length f time the ht wrk permit is valid (shuld nt be fr lnger then ne shift). The PAI shall inspect the area at least nce per day while permit is in effect t ensure the area is still fire safe. The PAI shall pst the permit at the wrk site. A fire watch shall be required by the PAI when ht wrk is t be perfrmed in a lcatin where ther then a minr fire might develp: Where cmbustibles are clser then 35 feet frm the ht wrk. Where cmbustible materials are adjacent t the ppsite side f walls r partitins and likely t be ignited. Shall be maintained fr at least ½ hur after the cmpletin f the ht wrk t detect and extinguish any smldering fires. Multiple fire watches may be necessary. RECORD KEEPING All cmpleted Ht Wrk Permits shall be kept n file fr a perid f at least ne year. All Ht Wrk Permit Prgram training recrds shall be maintained by the PAI.
