Purpse This manual is designed t be a framewrk t allw and encurage members f the JHU cmmunity t respnsibly use and perate lasers as utlined in the fllwing American Natinal Standards Institute (ANSI) standards: ANSI Z136.1, American Natinal Standard fr Safe Use f Lasers ANSI Z136.5, American Natinal Standard fr Safe Use f Lasers in Educatinal Institutins ANSI Z136.8, American Natinal Standard fr Safe Use f Lasers in Research, Develpment and Testing It establishes minimum standards fr safe peratin f experiments undertaken fr the purpses f recgnized scientific and engineering research and develpment. Where apprpriate, the language f these standards is utilized thugh nt explicitly cited. MOSH Maryland Occupatinal Safety and Health. Enfrces ccupatinal safety and health laws within the state f Maryland. MPE Maximum Permissible Expsure NHZ Nminal Hazard Zne OSHA Occupatinal Safety and Health Administratin. OSHA is a federal agency charged with verseeing ccupatinal safety and health n a natinal basis. OSHA may delegate their respnsibility t a state (See MOSH). PI Principal Investigatr The PI is the immediate supervisr fr the safety f university emplyees and fr students wrking in labratries under their cntrl. PLO - Principal Laser Operatr A PLO is any ALO wh has sign-ff authrity and respnsibility t apprve a Laser SOP. The PLO usually is the PI wh respnsibility fr perating a particular laser lab. 4 Respnsibilities and Duties University / Departmental Administratin is respnsible fr ensuring that the necessary resurces are made available t thse individuals wh are implementing this prgram. LSO (Laser Safety Officer) The LSO is appinted jintly by the Deans ffices f KSAS and WSE. This individual has the authrity and respnsibility t effect the knwledgeable evaluatin and cntrl f laser hazards, the implementatin f apprpriate cntrl measures, as well as t mnitr and facilitate cmpliance with recgnized r required standards and regulatins. Specific Duties and Respnsibilities include: Establishing and maintaining adequate plicies, prcedures fr the cntrl f laser hazards. Establishing and maintaining a safety educatin and training prgram t ensure the safety f laser persnnel. Develping plans t respnd t incidents f actual r suspected expsure t ptentially harmful laser radiatin. These plans will include: Prvisins fr prviding prmpt medical assistance t ptentially expsed individuals. The investigatin f the incident and the dcumentatin and reprting f the investigatin results t the HSC and HSE. Determining which persnnel shuld be evaluated by Occupatinal Health Services fr pssible medical mnitring Classifying and r verifying classificatins f lasers and laser systems n the Hmewd Campus. Hazard evaluatin f laser wrk areas with a fcus n minimizing risk, maintaining research quality t the greatest extent pssible under that restrictin. Assuring that prescribed cntrl measures are implemented and perable. This includes: Aviding unnecessary r duplicate cntrls, and recmmending r apprving substitutes r alternate cntrl measures when the primary nes are nt feasible r practical. Utilizing peridic audits, surveys and inspectins t cnfirm the adequacy and functinality f safety cntrl and measures. Reviewing and apprving the safety aspects f: All installatins, facilities, and equipment invlving Class 3B and Class 4 Lasers prir t use. When undergraduate students in teaching labs are invlved this als includes all Class 3A/3R Lasers. The purchase f all Class 3B and Class 4 lasers and Class 3A/3R lasers t be used by undergraduate students. Written SOP s fr all Class 3B and Class 4 laser systems and ther prcedures that may be part f the requirements fr administrative and prcedural cntrls. Fr lasers used by undergraduate students as part f a class labratry this requirement extends t Class 3A/3R lasers All prtective equipment, fr example, eyewear, clthing, barriers, screens, etc. The wrding f all area signs and equipment, t be in accrdance with the Standards wherever pssible. Assuring that the necessary recrds required by applicable JHU plicies and that gvernmental regulatins are maintained. Accmpanying regulatry agency inspectrs (e.g. OSHA, MOSH, FDA/CDRH, etc.) reviewing the laser safety prgram r investigating an incident and dcumenting any discrepancies r issues nted. 5 PLO (Principal Laser Operatr) / PI As the immediate supervisr PLO s are respnsible fr ensuring the safety f all individuals fr whm they have respnsibility fr wh may be expsed t laser radiatin greater than Class I. As a result, they shuld have detailed understanding and knwledge f laser safety requirements. Their specific respnsibilities include: Ensuring all persnnel wrking in their labs are prperly trained and supervised. Submitting plans fr laser installatins r mdificatins t the LSO fr review prir t perating the equipment in their areas. Cnfirming t the LSO that relevant staff (administrative managers, purchasing, building management, etc.) is aware f the pending acquisitin a Class 3 r 4 laser r laser system. Preparing and maintaining SOP s fr all Class 3B and 4 lasers under their cntrl. The SOP s shall be reviewed by the LSO befre implementatin. Maintaining recrds f individuals allwed and trained t wrk with lasers in their areas f respnsibility and prviding these t the LSO as requested fr medical surveillance scheduling and training cmpletin. Ensuring the safe and respnsible dispsitin f all unused, unneeded, but ptentially hazardus Class 3B r Class 4 lasers and cmpnents. Ensuring the prper reprting f incidents where injury did r culd have resulted frm an expsure t laser radiatin fr thse lasers under their cntrl. This includes implementing the JHU laser emergency respnse plan (see Appendix C and HSE 808), cmplying with JHU accident plicies (refer HSE 004), and ntifying the LSO. Implementing crrective actins t prevent reccurrence f incidents when infrmed that they have ccurred. Obtaining apprpriate medical attentin fr any emplyee, student, r visitr invlved in a laser accident (refer HSE-004). Prhibiting the peratin f a laser in their area unless there are adequate cntrls f all laser hazards t emplyees, students, visitrs, and the general public. Prhibiting the peratin f a new r mdified Class 3B r Class 4 laser installatin under their authrity withut the apprval f the LSO. Prhibiting the peratin f a new r mdified Class 3A laser installatin by students under their instructin withut the apprval f the LSO. ALO / LU (Authrized Laser Operatr / Laser User) The specific respnsibilities f ALO s and LU s include: Cmpleting laser safety training ffered r authrized by the Hmewd LSO befre wrking with any laser system ther than class 1 r 2. Using lasers safely while cmplying with safety rules and prcedures as prescribed by the PLO and the LSO. Being familiar with all applicable SOP s they are expected t perfrm. ALO s are allwed t wrk inside the NHZ s, while LU s are nly allwed t run experiments and prcesses utside the NHZ. LU s shall nt defeat system interlcks. When wrking inside within the NHZ while lasers are active, ALO s shall ensure that thers d nt enter the area. Reprting suspected r actual incidents where injury did r culd have resulted frm an expsure t laser radiatin they are perating /using t their supervisr r the Hmewd LSO. 6 Lab Instructrs - Thse individuals actively invlved teaching undergraduate and graduate lab curses where students are actively using cntrlled lasers shall Be qualified as ALO s; this includes Teaching Assistants (TA s) and Lab Technicians. Ensure that all students using the lasers in the labs have adequate training fr the hazards they will face in the lab in accrdance with HSE G05. Be present when students are using lasers unless frmally authrized by the LSO. Secure Lasers used by the students after each use. Reprting suspected r actual incidents where injury did r culd have resulted frm an expsure t laser radiatin f any persn in the lab which they are respnsible fr t their supervisr r the Hmewd LSO. Prhibit the peratin f a new r mdified Class 3A, 3B r 4 laser installatins by students under their instructin withut the apprval f the LSO Students - Students wrking with lasers Class 3A, 3B, r 4 as part any graduate r undergraduate class shall receive specific training as apprved by the LSO befre being allwed t wrk with the lasers. Students shall nly perate Class 3A r higher lasers with an instructr present unless specifically authrized by the LSO. Hmewd Laser Safety Sub-cmmittee The HLSS will cnsist f at least three members plus the LSO. The LSO shall serve as chair f the cmmittee. The three cre members will be selected as fllws: A PLO already sitting n the general HSC as appinted by the chair f the HSC. A PLO frm KSAS as appinted by the Dean f KSAS. A PLO frm WSE as appinted by the Dean f WSE. The HLSS shall be respnsible fr: Establishing and maintaining adequate plicies and practices fr the evaluatin and cntrl f laser hazards, including the recmmendatin f apprpriate laser safety training prgrams and materials Maintaining an awareness f all applicable new r revised laser safety standards. This includes cmmunicating directly with staff and faculty f the varius departments, centers and labs n the Hmewd campus. Reviewing and auditing accident reprts t verify cmpliance with applicable laser safety plicies. 7 Classes f Lasers All lasers with the exceptin f Class 1 with rated pwer f less than 1mW have the ptential t cause harm t the eye. T minimize the pssibility f harm JHU fllws the recmmendatins f the Standard with regard t tracking and mnitring lasers n the Hmewd Campus. The fllwing laser classes are cntrlled n the Hmewd campus and shall be registered using the Hmewd Laser Mnitring System Class 4 Class 3B Class 3A / 3R Class X Lasers nt specifically identified in accrdance with ANSI standards. These lasers may be under develpment as new lasing surces. The fllwing laser classes are nt cntrlled n the Hmewd campus, but users f these lasers are expected t understand the risks assciated with the lasers and fr Class 2 and 2M (ther than laser pinters) shuld receive Laser Safety Training Class 2 Class 2M Class 1 Class 1M LASER SAFETY MANUAL-REV1.DOCX Page 7 f 16

8 Emergencies, Accidents, Incidents and Revcatin All laser accidents and incidents shall be reprted in a timely fashin t the LSO. Failure t reprt accidents and incidents apprpriately (refer HSE-004) may result in revcatin f SOPs r permissin t use lasers. Accidents are defined as any incident where peple are injured r equipment is damaged in a manner incnsistent with the apprved SOP and /r nt directly assciated with the experiment. Incidents are defined as any situatin where: Individuals were ptentially expsed t laser light in excess f acceptable standards even thugh n injury was immediately nticed. Lasers and / r assciated equipment Did nt perfrm as indicated in the SOP Did nt perfrm as anticipated Perfrmed r was asked t perate in any way which may impact safety even if spelled ut in the SOP. Once incidents are reprted they will be reviewed by the HLSS t determine the rt cause fr the incident and what shuld be dne t prevent reccurrence. This review shuld be cmpleted within 10 business days. Cpies f the reprt shall be sent t: The PLO fr the laser The Chair f the Department and r Center Directr f the PLO The HSC HSE T the manufacturer f the laser and apprpriate regulatry bdies as apprpriate. 9 Laser Hazards and Cntrl Measures The fcus f the Hmewd Laser Safety Prgram will be t reduce the pssibility f harm by the expsure t the eye and skin f laser radiatin, r expsure t ther hazards assciated with lasers and laser systems during peratin, servicing, maintenance and strage when nt in use. Types f Hazards Laser irradiatin is nt the nly hazard that exists with lasers. When cnsidering the installatin f a laser the fllwing hazards shall be cnsidered as part f the installatin apprvals and SOP s. The list presented in Appendix A is illustrative and shuld nt be cnsidered exhaustive. JHU shall implement a hazard cntrl prgram that utilizes the hierarchy f cntrls detailed in Figure 1: Figure 1: Hierarchy f Cntrls frm LIA-Laser Safety Manual, 2007 (lia.rg) Substitutin -Substitutin is always the ptimal methd f cntrl. Eliminatin f a risk ttally ensures safety against that hazard. It is understd that risks cannt be eliminated, but they shuld never be greater than abslutely warranted (r than a level deemed acceptable). The lwest pwer lasers necessary shall be utilized in existing r planned installatins. If a higher pwer laser is selected because f planned future experiments, the SOPs shall be develped cnsidering thse hazards and experiments. As a substitute t direct viewing a laser beam within the NHZ, alternate means f viewing the beam during alignment shuld be cnsidered. Cnsideratins include: Charge Cupled Devices (CCDs) Web cameras Thermal imagers Flurescent papers and panels Dispsable targets Engineering Engineering cntrls are features and ther design characteristics f laser systems implemented t prevent the incidental ver expsure by blcking r preventing laser light frm escaping a defined path in an uncntrlled manner such that it culd cause injury t peple r damage equipment. Engineering cntrls shall be the first line f defense in preventing ver expsure t laser light. All engineering cntrls n the Hmewd campus shall be implemented using the best practices f the Standard except where alternate methds and/r materials cnfer an equivalent degree f risk reductin. 10 Persnal Prtectin Equipment (PPE) Eyewear is primarily cncerned with prtecting the retina as well as ther cular structures frm expsure t laser light. Laser glasses can prtect the lens f the eye frm ultravilet (UV) light, and bth the interir and exterir f the eye frm infrared (IR) expsure. Prperly selected eyewear will reduce the incident expsure t the eye t less than the Maximum Permissible Expsure (MPE) as defined by the Standard while maximizing Ttal Visible Light (TVL) transmissin. All eyewear used n the Hmewd Campus in cnjunctin with Class 3 and 4 lasers shall be reviewed and verified by the LSO. Fr details see Sectin 8 f the Standard. Other Class 4 (especially IR and UV) lasers may require ther PPE. The LSO shall verify that required PPE is present (see appendix A). Medical mnitring is required fr all ALU s f Class 4 lasers and is recmmended fr Class 3B lasers. This medical mnitring shall start with a retinal examinatin and visual acuity study prir t wrking with lasers. Upn leaving the university JHU students and emplyees shall be given a secnd study upn request. Administrative Cntrls It is recgnized that due t the nature f research wrk undertaken n the Hmewd Campus, it is nt pssible t implement engineering cntrls t design ut r remve all hazards. This is especially true where lasers must be aligned t ensure that an experiment is prperly set up. Training -All PLOs, ALOs, and LUs using systems cntaining cntrlled lasers shall receive training t ensure that they are fully cgnizant f the laser hazards they will face and what precautins they must take t ensure the safety f themselves and thers. Initial training shall be given prir t using the laser system with a refresher training being given nce per year fr class 4 lasers and every ther year fr class 3B lasers. Undergraduate students shall receive laser training when perating r using any lasers Class 3 r Class 4. There shall be specific training mdules tied t each class where the lasers are being used. SOP s - Prper training shall be supplemented with SOP s t ensure that ALO s take all necessary steps t ensure their wn safety and the safety f thers while aligning lasers r therwise wrking inside the NHZ. [See appendix B] SOP s shall anticipate the all knwn pssible uses f the laser system being described, including but nt limited t the fundamental emissins f the beam (frequency, pwer, rate), the specificatins f the NHZ, r engineering cntrls t be used in rder t minimize the need t resubmit the SOP fr review. Additinally SOP s shall be supplied t guide LU s t ensure that they can run experiments r ther peratins safely. ALU s and LU s shall immediately infrm the apprpriate PI and the LSO when they suspect that a SOP s has been vilated r when believe that an SOP is nt meeting its intended safety bjectives. Signage Signage has tw functins: Infrm spectatrs and ther casual users f a space f the existence f lasers. Act as a reminder t users f the space that lasers are present and what cnsideratins are necessary t ensure safety. All cntrlled laser installatins n the Hmewd campus shall be indicated with the apprpriate signage as recmmended by the Standard. Laser Registratins - Fr a minimum f five years after dispsal (see Laser Acquisitin, Transfer and Dispsal Sectin) (LSO). SOPs Fr a minimum f five years after the laser facility has been taken ut f service (see appendix B) (PI and LSO) Medical recrds (fr thse under medical surveillance) fr a minimum f tw years after the individuals invlved have left the Hmewd campus (Occupatinal Health). 12 Laser Acquisitin, Transfer, and Dispsal Laser Users have an bligatin t ensure that lasers are used safely frm befre purchase thrugh t final dispsitin and dispsal. On the Hmewd campus all Class 3 and 4 lasers shall be tracked and mnitred using the HLMS. All ther lasers shuld be purchased by the users with the understanding that they are respnsible fr ensuring that the lasers are dispsed f prperly as electrnic waste t prevent injury t thers. Acquisitin r use f any new Class 3 r 4 lasers n the Hmewd Campus shall be reprted t the LSO The LSO shall review all new laser installatins t: Evaluate the apprpriateness frm a laser safety standpint fr the planned applicatin. Cnfirm that the lwest pwer necessary fr the intended use is purchased. Determine what engineering cntrls shuld be implemented as part f the installatin. When purchasing Class 1, 1M, 2, 2M and 3A/3R lasers the user shuld have a plan fr their prper use and eventual dispsal. Once a new Class 3B r 4 laser is n campus it: Shuld be registered with the HLMS within 24 hurs. Shall have preliminary (filed but nt necessarily apprved) SOP in place prir t being energized, this includes: The names and training status f all certified ALO s and LU s. Establishment f the NHZ. Assurance that all persnnel using the laser system have adequate PPE. Shall have an SOP apprved by the LSO in within tw mnths f being first energized. Shall have the SOP updated and reviewed n an annual r mre frequent basis. When a laser is remved frm service it can be: Placed in safe secure strage under the cntrl f the PI. Placed in safe secure strage under the cntrl f the LSO fr the purpse f reassigning t ther PI s and r Departments. Sld r ther dnated t ther respnsible laser user utside the university. Dispsed as excess prperty. In all cases lasers shall be handled fllwing best practices fr lasers and ther electrnics and in cmpliance with JHU labratry equipment clearance plicy (HSE 810).
